A Regulatory Turning Point
On April 24, 2026, the FDA awarded Commissioner National Priority Vouchers to Compass Pathways (COMP360 psilocybin for treatment-resistant depression), Usona Institute (psilocybin for MDD), and Transcend Therapeutics (methylone for PTSD). Compass separately confirmed its COMP360 NDA remains on track for Q4 2026, after two consecutive positive Phase 3 trials. Texas is advancing state legislation to authorize psychedelic-assisted therapy clinics.
But the regulatory news cycle misses a more fundamental question: what is actually happening psychologically when these compounds work?
How Psychedelics May Interrupt Addiction
The dominant proposed mechanism is disruption of rigid, self-reinforcing thought patterns — what researchers call cognitive flexibility or loosening of psychological rigidity. Psilocybin and similar compounds act primarily at the 5-HT2A serotonin receptor, temporarily increasing neuroplasticity while reducing activity in the default mode network — the neural substrate most associated with self-referential thought and rumination. In lay terms: the story you have been telling yourself about who you are and what you are capable of becomes, briefly, more fluid.
The Therapeutic Container
This is where the assisted in psychedelic-assisted therapy matters enormously. The clinical model involves extensive preparation sessions before the drug experience and integration sessions afterward. In the addiction context, integration sessions help a person examine automatic patterns that have structured their relationship with substances — and, while the window of increased neuroplasticity is open, build new cognitive and behavioral frameworks. This requires skilled therapists. It is expensive, time-intensive, and completely unlike most outpatient behavioral health care in the U.S.
Where the Field Disagrees
Not everyone is convinced the mechanisms are as clean as advocates suggest. Some researchers argue the mystical or peak experience common in higher-dose psilocybin sessions is doing the heavy lifting. Trial populations to date have been overwhelmingly white, well-educated, and psychologically stable — conditions that may not reflect who most needs addiction treatment. Scalability remains a real question: even if COMP360 receives FDA approval, the trained therapist infrastructure required does not yet exist at scale.
MDMA path is further complicated: FDA declined a 2024 NDA for MDMA-assisted PTSD therapy and required a new Phase 3 trial. Psilocybin may reach approval first.
