An Addiction Clinic Just Built an AI Companion App for Everyone
Pelago spent seven years selling itself as a specialty substance use clinic. Its new AI coach, Sona, is a different business wearing the same trust.
Pelago spent seven years building a name around one narrow promise: specialty telehealth for compulsive substance use — what clinicians call substance use disorder — covering alcohol, tobacco, cannabis, and opioids, sold to employers and health plans who wanted something more clinical than a general wellness app. Behavioral Health Business reported on July 8 that the company shipped something else: Sona, an AI voice-and-text companion marketed as general “mental wellness support,” open to anyone, not just people already enrolled in Pelago’s substance use programs. It’s a real, live app, not a pilot.
This isn’t product expansion. It’s a specialty clinic hedging against its own market.
CEO Yusuf Sherwani told BHB the move reflects “a really marked shift of heightened concern about healthcare affordability and then specifically behavioral health affordability.” Translated out of investor-speak: employers are squeezing what they’ll pay for a niche addiction benefit, so Pelago built a cheaper, wider product to sell alongside it. Sona remembers what you’ve told it across sessions, offers coaching between therapy appointments, and is supposed to hand off to a human clinician if it picks up a crisis signal in what you type or say.
That handoff promise is the whole ballgame, and it’s also where the comparison to Pelago’s old business breaks down. An AI trained to flag risk and route to a real person is a reasonable design — Woebot and Wysa have run versions of it for years, built as mental health products from the ground up, with safety review baked into the founding premise. Pelago was built to treat addiction specifically, which means Sona inherits its parent company’s existing patient base and its parent company’s incentive to keep engagement metrics climbing, not necessarily to keep any one person safe. Pelago raised $58 million in 2024 on the pitch that narrow and clinical beats broad and shallow. Sona is the opposite bet — wide, ambient, and monetized on habitual use of a companion app, the exact pattern that’s already drawn lawsuits and regulatory scrutiny elsewhere in the AI-companion space.
If you’ve actually used Pelago to work on drinking or opioid use, the pitch to also open its new mood-tracking bot between sessions is going to land less like a clinical add-on and more like a company that already has your intake history finding a new surface to keep you on. Recovery care runs on the assumption that the people on the other end of the app are optimized for your outcome, not your daily-active-user count. A specialty provider quietly becoming a general-purpose AI companion company doesn’t automatically break that assumption. But it asks you to extend the same trust to a product built for a different business model, without ever asking you outright whether you want to.
