Your Xanax Dose Is Not a Detail. Finnish Researchers Just Showed It’s the Whole Risk Curve.
If you take Xanax, Valium, Klonopin, or Ativan, the question that matters most for your safety was never “should I be on this” — it’s “what’s my dose, and how long have I been on it.” A University of Turku study of 48,124 benzodiazepine users, followed for five years and led by Hanna Särkilä, put an exact number on that intuition, and the number should change how both doctors and patients talk about these drugs.
Below a threshold, benzodiazepines barely move your mortality risk. Cross it, and the risk of dying by overdose more than triples.
Here’s the plain-language version of what “dose-dependent mortality” means: it’s not that benzodiazepines are dangerous or safe as a category — it’s that the danger scales directly with how much you’re taking. At low doses (under 10mg diazepam-equivalent, the standard way researchers compare different benzodiazepines against each other), the study found no meaningful increase in overall mortality, though external-cause deaths — accidents, overdoses, suicides — rose 17%. At medium-high doses (10 to 29mg), overall mortality rose 58%, and external-cause mortality rose 129%. At very high doses (30mg or more), those numbers climbed to 168% and 304%. Suicide risk at medium-high doses was 2.43 times higher than at low doses; overdose risk was 3.34 times higher. People taking three or more benzodiazepines simultaneously — a combination that happens more often than most patients realize, especially across multiple prescribers — saw mortality risk rise 212%.
Who’s actually prescribing this, and how that’s shifting
The dose data lands the same month as a second study that explains part of how people end up on higher doses in the first place. A JAMA-adjacent analysis in Annals of Internal Medicine, led by Mark Olfson, tracked benzodiazepine prescriptions to adults 65 and older from 2015 to 2024 and found something specific: dispensing fell 18% overall through the decade, a real public-health win driven by years of deprescribing campaigns — but that decline stalled out after 2020, and the prescribing landscape underneath it has quietly changed shape. Long-term care pharmacy dispensing rose sharply. Advanced-practice clinicians — nurse practitioners and physician assistants, who typically carry larger patient panels and less time per visit than a primary care doctor — now write 30% of these prescriptions, double their 2015 share, while primary care physicians’ share fell from 68% to 57%. The South accounts for 43% of all prescriptions nationally, more than double the rate anywhere else in the country.
Put those two studies together and the picture sharpens: the population most likely to be on benzodiazepines long enough to reach dangerous doses — older adults, often prescribed for sleep or anxiety and rarely re-evaluated — is increasingly being managed by clinicians with the least bandwidth to run the kind of careful, incremental tapering that gets someone off a high dose safely. Cold-stopping is not an option here; benzodiazepine withdrawal, unlike withdrawal from most other drug classes, can cause seizures and is genuinely medically dangerous, which is exactly why the tapering itself needs a level of clinical attention the prescribing trend is moving away from.
Mother’s Little Helper, then and now
This is not benzodiazepines’ first reckoning, and the history is worth fifteen seconds because the pattern is exact. When Hoffmann-La Roche launched Valium in 1963, it became pharmaceutical history’s first $100-million brand. Prescriptions rocketed from 500,000 in 1965 to 29 million by 1970 to a peak of 88 million in 1978 — most of them written for women, marketed explicitly as relief from the tedium of housework and domestic anxiety, cemented in the culture by the Rolling Stones naming it outright in “Mother’s Little Helper” in 1966. It took until 1975 for the DEA to classify benzodiazepines as controlled substances at all, and the FDA didn’t require a class-wide boxed warning on abuse, misuse, addiction, and withdrawal until September 2020 — forty-five years after the government first acknowledged these drugs needed controlling, and fifty-seven years after the first prescription was written. Every decade in between, the same drug class got quietly re-normalized under a new brand name and a new marketing frame, from Valium to Xanax to Klonopin, each one arriving with the same reassurance that this generation’s version was better understood, safer, more precisely dosed.

There’s a second, faster-moving version of this same dose problem happening entirely outside the pharmacy. Designer benzodiazepines — etizolam, flualprazolam, flubromazolam, and others never approved for medical use anywhere — have been showing up in counterfeit prescription pills and pressed into fake Xanax bars sold on the street, often at doses the person taking them has no way to estimate or verify. Standard hospital drug screens frequently don’t detect them, which means a physician working an overdose has less information about what’s actually in a patient’s system than the Finnish researchers had about their own study cohort’s precisely-logged prescriptions. When federal scheduling catches up to one designer compound, chemists shift to the next unscheduled analog within months — the same substitution pattern that has played out repeatedly across other drug classes when enforcement targets a single named substance instead of the market producing it.
The community holding this alone
There’s a reason the loudest, most detailed conversation about benzodiazepine withdrawal doesn’t happen in a doctor’s office — it happens on Reddit. Researchers who ran a formal content analysis of r/benzorecovery — a large, active peer-support community, examined here at the theme level only, with no individual posts quoted or identified — found recurring conversation clusters around protracted withdrawal (often shortened in the community to “PAWS”), fear of “kindling,” where a second attempt at tapering produces worse symptoms than the first, and a deep, specific distrust of the psychiatrists who wrote the original long-term prescription without ever explaining what coming off it would involve. Mutual-aid tapering schedules — liquid titration, compounding-pharmacy workarounds, spreadsheets tracking milligram reductions in increments most doctors would never propose — get built and shared peer-to-peer because the formal medical system, on this specific drug class, has largely left patients to become their own pharmacologists.
